研究摘要

Abstact:

Orthokeratology (OK) was initially on the market as an alternative option for patients who wish to avoid day-wear contact lenses or glasses and hesitate about refractive surgery. Studies later proved that over-night orthokeratology has the effect of slowing myopia progression. Orthokeratology is currently used in Taiwan mainly for myopia control in school children. Animal studies showed that the growth and development of the eyeball is related to the refraction of peripheral retina. Peripherally hyperopic eyes have stronger tendency to elongate in axial length, while peripherally myopic eyes have less tendency to do so. The mechanism involved is proposed that the corrective lens flattens the central cornea and steepen the mid-peripheral cornea, which causes a more plus refractive correction relative to the center and induces spherical aberration (SA). The eye becomes peripherally myopic, hence less tendency for axial elongation. Clinical studies showed that the effect of OK in slowing myopia progression is less significant in patients of low myopia, and more pronounced in patients with medium to high degrees of myopia. Our study found that amount of positive spherical aberration induced with OK is related to the amount of myopia corrected. The higher degrees of myopia corrected results in higher amount of spherical aberration induced with OK. This leads to the more pronounced effect of slowing myopia progression. Based on previous and our studies, we designed a monofocal aspheric soft contact lens with fixed amount of positive SA to simulate the SA induced by OK treatment of moderate myopia. Potential advantages include hygiene, convenience, higher range of treatment, predictable effect, less decentration-induced coma. We will conduct a clinical trial to evaluate the effect of this new monofocal aspheric soft contact lens in slowing the progression of myopia.
